Need Program and Script - Db, PHP, MySQL

Looking for someone to program a simple script that will take this data:

And generate pages from it.

So the script will work link this:

Main page - All 50 US states

Second Pages - Cities - based upon state and city from csv file above.

Third Pages - List of businesses in that City i.e - New york, New York

Fourth Pages - Business Info Listing with all the info in the csv file for that listing.

Want this in PHP and to use the MySQL for the database.

Want a backend that us secure and can edit the meta tags for all the pages that are generated, Update a listing in the db, add a listing to the db, delete a listing in the db and able to put ad codes into the script generated pages.

Have a sample I can show you , but want it updated.
