Need PHP-SQL coder ASAP

    Hi,

    need someone to code a PHP-form that writes the text input into a SQL database. The form wil have ~20 formfields, some of them will allow multiple choice boxes, some are free text and some have a scrolldown-menu. 2 are "must" fields that must be clicked to send the formular (like "You have to agree...").

    I can give you a html-layout of the formular, you do the PHP and SQL part.

    You see, it's not a big thing, but I need this done the next 24 hours.
