Buying Need PHP programmer to fix PHP/Ajax Bootstrap form - $15

    I am looking for an experienced PHP coder to fix a couple issues on a PHP/Ajax Bootstrap contact form. It works fine except when the user leaves the input fields blank, the form still submits a 'successful submission'. It needs to be configured so that when someone leaves a input field blank, the submission returns an error, reminding the user to complete the fields. All the scripts including validation are there. I think, I might have missed a path to a file or something.

    Also, the form results received in my inbox shows the email address for the 'from' headers, it should actually display the name of the person submitting the form.

    I will pay $15 Paypal for this fix. Thanks.
