Need Paypal Subscription form created

    I am looking for some to create a form for a user to fill out with the following:

    $12 Monthly Paypal subscription

    Name
    Email
    URL
    drop down menu with $20 optional component
    Topic


    The form should go to Paypal for subscription creation: with or without the additional $20 item then IPN back to my site to another form for the user to fill our more information which is then send to me via email.

    Is there some reason you want to do it exactly this way?

    Could you not let them subscribe, return and fill in all the information at one time?

    Do you need this info in a database? Or just emailed to you?

    I do want to do it pretty much this was. I do not need the info in a database for now but will change to a database format in the future. I just need to get somethign up and running for now.

    Still looking!
