NEED NOW! Design of sqeeze page (know what I need)

    I need a design of page similar to this one: bestseniorcareonline.com - not a copy. Will supply photos, logos and text. Need design and HTML.
    Communication via yahoo/icq/gmail, need to design it fast. There would be no pre-payment unless you have a iTrade 20+ on design
