Need Logo for $5, less than 24hours

hai designers,

Due to low budget, i only mange to pay $5 for this job.
I need a logo for my new site.
Requirement:
1) white background
2) size : 350px*75px , 400px*75px ( same logo, just resize it for me )
3) i need slogon under or above the logo:
- im not a wrist specialist or doctor, but a patient with wrist problem ( short this sentence if you got any idea )
4) match my site theme : mywrist.info

Example : http://mywrist.info/wp-content/themes/ProStart/images/logo.png
i will be very happy if got better looking logo than this example.

Payment via paypal, i choose the design in less than 24 hours.

Yes, my site will provide information about wrist problems

Comments:
didrapost - icon in the logo does not match, and looking more than one color in font.

telegendary - nice, but the logo is too small

guedda - interesting, will be better if include small icon like other submissions

filebrigade - have all my requirements, nice. Just change the slogon a bit : Not a doctor, but a patient with a wrist problem.

more appreciated if all submission provide a "preview" like how guedda did, easy for me choose.

I am going out now, i will choose the winner after come back, approximately 10 hours. Good luck guyz.
