Need JS Modifications & Semantic CSS on Quote Engine Script

    I posted my project on Elance at the link in the bottom of this post.

    Here is a description of my project:

    I currently have a table-based (2 step) web form that connects into a popular health insurance quote engine and produces quotes based on information inputted.

    There are two things I need to do.

    1. I need to add fields to my form to allow for additional information collection.

    2. The current form is table-based. I want to convert the table to beautifully valid CSS that is cross-browser compatible. I also want to style the form to look and feel better and function better. The use of a small ajax function might be necessary as well.

    If you are interested, I can hire through Elance or not - if you prefer other method. However, I prefer Elance. Also, I require you show me some form of previous work that displays the project requirements (CSS & JS usage together).
