Need Help with php/java

    Hi
    I am trying to turn 3 <option ...> codes into one field of <checkbox ...>. So in other words... instead of people having to select from 3 drop down menus, I want them to be able to select the 3 options on one set of checkboxes.

    I already attempted it... but to no success.

    Also... I don't want them to be able to select more than 3 boxes.

    Anyone capable of fixing it/doing it, please PM me.
