Need help with online form - $6.50 via PayPal

    Hello, I need help with the back end of fairly simple form I created. The front end is already completed. I just need the form to send the data to my email when someone clicks the submit button.

    If completed today, I can pay $5 via PayPal.

    If you can have this completed in an hour or less, I can raise the payment to $6.50 via PayPal.
