Hello Sir, If you just need the FORM done,then its fine.. But if you need it to function and add your user to the database or make some payments,then its impossible to do with 300$ even.. As far as I see,it needs a custom component. We did almost similar kind of component for one of our client 3 days back.. Well, if you just want to catch the data entered,then I can do it for 35$ ..
