Need help making a Form Submission for my webapage

Hi,

Ive made a form with a submit button for customers on my website to fill out their information.. but how exactly do i receive the results? Can someone explain how i can set this up?

 

You need a file to process the action of sending the results....

I already have the form made up but need to get the submit button to work.. if anyone can help me what to add ill pm the form to you.
