Need Help Creating a Form for a website - will pay for help

I just need a simple form users can fill out, and once it's filled out, the info is emailed to an email address. If someone can show me how to do this, great. If you want to do it for me for a little $, that's fine too.

Here's what I need in the form:

Workshop:* (drop down list: “Business” “Family” “High School” “Elementary”
Address where conservation kits were used:*

Email Address:
Water Usage Before Workshop:*
Water Usage After Workshop:*
