Need form in image coding

    I have a membercard that I need visitors to fill out directly on the monitor. Then they should be able to print it. Maybe it shoud be done in flash?

    In short:
    - Visitor arrive at the page
    - Visitor fills in details
    - Visitor clicks a print button and the card get printed.

    Here is an example of the card, I have it in better quality as pdf:

    it can be done without flash as well using php and a bit of photoshope i guess...
