Need Database/Form Fix

Hello,

I recently acquired a site that had a submission and contact form included and tied to a database. The database was tied to the previous server name and I don't have the database. I am looking for someone to fix the 2 forms (they're already designed and made, just need them to work properly).

do you need the DB to be constructed from scratch based somehow on the 2 forms you have or do you need to point the script to a new DB server.

 

I will need a new database created as I don't have one established at the moment.

so I'd need the database setup and the forms tied to it to work properly.
