Need Custom Order Forms

    This is a fairly simple project, all I need is 5 custom order forms that would use the PayPal API. The forms would only vary slightly with a few different fields, so it's really nothing too hard. The only thing I need done besides that is adding support for iDevAffiliate. Please PM me quotes with portfolios. If you do not have a valid portfolio, you will not be considered.
