Buying Need Contact Form Re-Coded

    I have a contact form that is already styled, I just need a bunch of extra text and check boxes, validation for a pssword and confirm password boxes, and an upload to handle images. The alignment of labels and boxes needs to be changed slightly, but that should be it CSS wise.

    Let me know if you're interested and I'll send you the files so you can quote me. I need this done today if possible.
