Buying Need contact form configured to a PHP script. Will pay $15.

I quickly need someone to configure a contact form for a client. All the fields and drop downs have been inserted. Just need it to be configured to a PHP script and include a captcha verification box on the form where indicated. The form submission will be redirected to a thankyou page. Will pay $15 through Paypal after confirmation of the form working.
