Buying Need contact form coded

    Just like the title says, I have a contact form laid out in Photoshop and need it coded (needs to be php). PM me your GTalk / MSN name and what you charge, I'll message you on either one to get the project started.

    Only PM me if you can do this NOW since its a quick thing.

    The form is 10 questions (7 text fields and 3 radio buttons)
