Need changes done in php web form

    Hi all

    I have this one form we created for our client.

    we are having some problems with it like spam and bots filling out forms which are not useful to my client at all. Here are the things we will like to see changed in this web form



    First- Please DO NOT let the forum SUBMIT to our e-mail unless they completely fill out the fields with asterisks. As of now the forum does not do this, therefore the asterisk means nothing.
    I would like asterisk and limitations added to the drop-down fields of:
    -Phone service
    -Customer birth date
    As of now the forum allows submissions without these fields clearly filled out.

    Second- I would also like the zip code field to be limited to numbers and only 5 digits.

    Third- Please make the phone service drop-down box and birth date drop down box bigger that way these fields are not cut in half. I would like theses boxes to be clearly visible.

    Forth- I would also like the 1st responder field to be required before the form is submitted. (add an asterisk and restraints on submission)

    Fifth- we will like to see CAPTCHA installed before the form is submitted to our email.
