NEED: Auto email coding & attachement to web form

    I currently have a simple web form setup that inserts 4 values into a database:
    email, subject, city, state.

    I am using this to keep track of incoming emails on various subjects, but when I hit SUBMIT, I want it to send an email to the email address entered in the form with the subject as the subject entered.

    In the body of the email, I want it to say "Hello. Regarding [subject] in [city], [state] I have attached the following documents.

    Thank you,
    Tom Jones"

    As well, I need to attach three documents (doc1.doc, doc2.doc and doc3.doc) that will always be attached to every email sent out.

    Anyone want to take this task for me? It shouldnt be that difficult as I have it setup on another advanced from using dynmaically created PDFs but I dont have the time to work on it myself.

    PM me with questions and YOUR COST. I dont have a budget but its a matter of time vs. money.
