I need a simple form handler that will capture all of the data on this page. As part of this project, I'll require javascript code to force the completion of the form where asterisks are located. Or this can be done in the asp to check for all the data before adding the data to the database. An error message must be delivered to the applicant to inform them of what they are missing. The data needs to be dumped into a mysql table and then emailed using cdosys in a useful format to the administrator. Lastly, I'll need another page to display the data using the exact same form layout for administrative use. For the data display, you may ignore all elements on the page above and below the <form> </form> tags.
