Do you mean how you should do Local SEO? Well firstly, you should have your city or location in the title tag along with your keyword: "Dentist in New York - *Dentist Name*". Yeah, much like Helge said and his blogpost highlights (great post by the way) you want to target keywords that are saturated with your intended audience and area.
