Need a Wordpress Plugin to Add Custom Fields to Comments

    I don't know why I've had to search for a solution on this for two hours...but for some reason I can't find a way to simply add extra fields to my comments form in Wordpress.

    I found one old plugin that I guess was left to die back in Wordpress 2.5.1 but other than that...nothing.

    Can someone point in the right direction to add more fields to my comments form? I'm sure there must be a lot of people that need to do this so the answer must be out there.

    The plugin you have suggested does not work with Wordpress 2.6+.
