Hey, I need a good programmer to build me a quick online survey. I could do this myself, but I'm in the process of trying to find good programmers to outsource work for my firm to. The survey will have 3-5 questions, each of which is multiple choice with an "Other" choice that has a text field. The survey answers would simply need to be emailed to me in a nice and neat format. No need for a database. The form also needs to validate, so you should know a little about javascript to do that. You will also be responsible for design. It doesn't need to be super graphical, but something nice and professional. If you cannot do the design, I'm ok with getting someone else to do that part. PM with quotes please!
