Need a simple HTML + PHP Form implemented into Wordpress

    Hi there,

    I need a simple for implemented into my wordpress theme and I need it quickly. I had someone working on it but they flaked out on me and this was meant to be due 2 days ago, so only ppl that can get this job done quickly please apply.

    Its a basic registration form for a conference. I just need the details sent to a nominated email address. The form contains only 16 fields mainly text and a few checkboxes. I can pay via paypal or can offer a trade for my design services:
