Buying Need a Simple Custom Form w/ Submission Coded

    I need a form with about 8 fields created. And when someone submits this form I'd like it to generate an e-mail to my account.

    I have an example of the form I'd like to create, and you can copy/duplicate/clone the styles exactly.
