Need a script for Simple Enquiry form

A simple enquiry form with the following fields
- Name
- E-mail address
- Contact No.
- Enquiry

Submit and Reset button

when you hit the submit button, it has to send a mail to the specified e-mail address in the same format.

And after submittion of the form, we have to get a "Thankyou" page.

I shall pay for the script. Its very urgent.
