Need a quick PHP Fix - $10

    Were having problems with a PHP script. The problem details are below:

    The address for the script is:

    The script is almost working perfectly but the issue is with the email that is sent to me (as a lead) does not have the correct answers that were submitted by the user.

    When the form is complete and submitted, the questions selected are not saved correctly. The script only "remembers" the default (or first) answer. For example, if you select any state other than New South Wales, the answer email always shows New South Wales.

    Is it possible to fix this?
