Need a PHP/MqSql Job Done (Upload/Submission)

Hey, I need some PHP/MySQL work done (assuming this is the best way).

What I am hosting is a site with maps for PC Games, so there must be a form on a page where users can submit a map. It needs to include a submission for a screenshot of the map, name of the map, map size (sm, md, lg), and an upload box for the map to be uploaded on to my server (which you'll need to figure out). The file size limit should somehow be set to 10 mb (however, I would like to be able to change it. Also, I would like to somehow allow only the upload of certain file types).

Then, I need to be able to sort the database using different options: Most Popular (max # of downloads), Newest, Oldest, etc. and display it on a page to my users.

Please let me know if anything is unclear and what the cost will be. It is a pretty basic job but the only part that I'm not familiar with is allowing users to upload maps to my server securely.

I've got a pretty low budget, so I'm not looking for anything unique/custom. Free scripts compiled from the web work fine, I'm pretty much looking for someone to find the scripts and piece it together.

Repentless, do you have any examples of your work? Bobby have a much higher iTrader and Rep. Although I am looking for a good price, I am willing to spend slightly more for a quality programmer.
