Buying Need a PHP Guy for custom forms Simple Job - $20

What I need is for you to create a very simple booking form, which must be linked through paypal.

I need 2-3 forms.

One for Course Title (drop-down menu)
One for Course Dates (drop-down-menu)
and finally a confirmation displaying like;

"You have selected "course_title" on "course_date", this course will cost "course_cost" "

PAYPAL BUTTON HERE to PAY

The information from the selected course and date, should be linked with paypal, so if someone paid, then in the paypal confirmation email it should display that information.


I will need a very simple admin panel to change / setup
Course Titles
Course Dates(these must be linked to the course title, and they could appear like
1st April - 5th April
5th May - 17th may
and so on.
Course Price (GBP)
and an option to setup paypal email.
