Need a PHP form completed .... $7

I don't have the time or skills to complete this form and get it to work. Will pay $7 to the first person to get it complete and working as I want it. Only one person will be paid, and that only after I have approved the work.

what is it going to use to send the actual email? php's sendmail or smpt?

 

I'm not sure... whichever works I guess? Probably PHP sendmail... ?
