I might be able to do this. A simple pop-up calendar using AJAX where they click on a date, and than that's added to the form ? EDIT: Thanks to the one saying I must read.. EDIT2: What I meant to say was, it's not an event calendar, right? What is the purpose of mailing it and then storing it, in what kind of environment will it work. That's what I meant.

Note that ajax calendars and memerships scripts already exist all around the web (dhtmlgoodies.com for example) you are not requested to write all this from scratch. An email to the member recapitulating his information. A logout button some more things will have to be done but i d like to start with the fondamentals, a login page, and the form.
