need a custom Upload script with progressBar (using html/php/perl)

    1st of all, plz do not suggest me Uber, mega or xupload

    i need a very basic one & i'll be greatful if someone write for me.

    the html form shud be like this:

    Name: <textBox>
    Description: <textArea>
    Language: <dropDownList>
    Choose a File: <fileBrowser>
    <submitButton>

    there will be a process.php file which will process the file & later it'll add data (name, desc, file name....etc) to database (mysql)
    I'll write this php file myself later

    Now wht i need...plz write a script (maybe using perl/cgi) so it'll show Upload progress after clicking submit button & after upload complete it'll pass Name,description,lang,File name & other variables to that PHP (progress.php) file

    Man...plz read post before reply. At 1st line of my post, i wrote no uber, mega, xupload or this kind of advance/complicated script...i need a custom script..very basic & easy
