Need 17 Form Web Application Designed $100~$300

    We need a 17 form we application designed. Approximately 35 fields per form. All fields are text fields except a couple places where they have to upload and download files. All forms enter data into or display data from a MySQL database. The database is already desgined and implemented. We only need the forms created to read info from and input info into mySQL tables.
