Name and Email fields on my blog

Hi,
I want to add Name and Email fields to my blog so I'll start to collect an Email list.

I have no experience with HTML programing so I dont mind paying a reasonable amount for this.

I want to have the option to:

1. change the target email (the email the information is sending to).
2. change the Button "send" (shape and description).

yes you can do it by your own.. just try what "itcn" has suggestd.. also you can go for other contact form scripts available online if not satisfied with this one..
