    You definitely need to change that banner.. Looks way too cheesy. You also need to add more information about what you have to do to get the "free" items.. I want to know what I am signing up for if I am going to sign up for something like this..

    I agree, change the header to something more professional. Also the headers are slightly hard to read as they have light gray backgrounds with white text.

    Other than that it's fairly clean and simple which is good.
