My Co-op Ad Network Account have been taken out of the rotation. HELP

I have the ads on my main page www.phppod.com and the forum www.phppod.com/forums

Everything was OK for about 2 weeks but now I keep getting message "Your ads have been taken out of the rotation" and the system looks for ads in all weired places like http://www.phppod.com/forums/archive/index.php

Also it looks for the code in every page like http://www.phppod.com/forums/forumdisplay.php?f=8 but I have it in the footer which is displayed with every page.

Any help will be greatly appreciated

 

Looks like you may have been penalized? Did you do anything that could cause this penalty, like hiding links or cloaking?

 

Nope. I haven't done anything like that

I see the coop as on the homepage, but not on the forums; might want to double check that
