Hi I do not think so, once it is blocked, first is the challenge of getting it back which is near zero. If you use different information and manage to get a new Adsense account, using it on your same blogs is giving you away to Adsense. They will know who you are and ban you again. What I suggest is try your best to appeal, if you click your ads, tell them that, be as honest and sincere as can be. If appeal fail, look out for new advertising agency like Fka200 point out Adbrite for eg.
You can get your name and address back. but, can try with another persons name. Start with a blogspot unique blog (with no ads before) and later ask google directly whether you can include the previous sites pretending that you bought the sites from its previous owner. It is ugly but, it is the way forward for you.
you are in no rush here, are you? if you sign up fo a new account, that will take you few days. if you write to google and wait for answer, that will take weeks. if you had your account banned, then it is obvious that you did something which google viewed as violation of TOS. should have thought about that ahead of time. its hard to get another adsense account and keep it for long. even if you sign up for a new account, google WILL find out and ban that as well. only way you can get away is by using some other person/falimy member's name with different address
