HTML table code is fine providing we host the code and it's just non-intrusive static valid HTML code. For banner it's same price, $10 / month.

Yes your banner would be in a rotation with 3 other private ads and network partner ads, network partner ads get display 40% less than private ads and all private ads get equal views. The website is currently doing just over 20,000 page views a day so plenty of impression to go around.
