I have a quick and easy job (if instructions are followed correctly) that I need done within the next 24 hrs (MAX), let me explain everything. Download this word document click here We have a word document which is a 14 pages form, we want this whole word document converted into HTML based form, database table fields and MS-Word bookmarks created for all the fields. Basically, we want this form to be a web based form, once its filled in it will insert all the entries to the database and then generate (we will do this part) the word document filled in with the entries. Here is what you need to do: 1. Convert the MS-Word document into HTML based form, (please check this link, we have done about 6 pages, you need to follow the same design and structure, and you can resume working from there) 2. Once you design the form in the same structure as we done, then you need to rename each form fields (remember the form field names) 3. For each form field create a table field (this needs to be done just once) 4. Once submit button is pressed it need to insert into the database table you created. 5. Finally, edit the MS-Word document and give each field a bookmark name with the same bookmark name as the database table field. To create a bookmark, you just click on the field and press CTRL+SHIFT+F5 and give the bookmark a name. The max budget for this project is $50.
