Missing Adsense Check - What happens next?

    Does anyone know the policy for resending the missing checks? I have not got the one they sent 4 months ago. Do they just add the missing check to the new one or they have to have me sign some document-- claiming they are lost.

    The last time an offcial check was missing, I had to file 2 papers and got them notorized.
