MFHS v1.2 Uploading Form...Need to Change its style [URGENT 2 hours]

    Need a job done on MFHS...
    restyling of the uploading form...
    this includes changing the position of stuff not just changing the color of the form...
    basically this is what is to be done:

    shouldnt take more than an hour if he knows what hes doing
    need it done within the next two hours
    its really urgent..

    NEED IT URGENTLY!

    requirements:

    1 - should look like upload form design http://img10.imageshack.us/img10/2039/needthisdone.jpg
    2- when add more option is clicked on the blue background area of fomr should expand in height
    3- should be integrated into mfhs script. server info will be provided
    4- Needs to work!
