Try to write your tags in a way that they will be human friendly. Dont stuff your title or your description with tons of keywords.
Create a title with one or two keywords, a title that will make sense. Use the same "guideline" for the description tag.
    IMO :p

There is no any procedure or rules just keep in mind these points:

    1) As Google considers only first 65 characters of the title page, but other search engines can do consider more than that also, so your title within 65 characters to 100 characters is, ok.

    2) Your Meta description should not be the same as title.

    3) Meta description need to contain targeted keywords, but not keyword stuffing is advisable.

    4) Meta description of the page should be well in grammar as well as spell mistake free.

Keep in mind... and go ahead.

    title lenght: 500 characters
    description lenght: 250 characters
your title should include the keywords for which you want your site to be best optimised. they should be user friendly too.
as far as description is concerned, it should be such that describes your site well. what all services do you provide and can include some keywords also.

create title not more than 65 character and not more than 150 characters for meta description. Just make sure that title and description is readable and not full of keywords.

    - Start with your keywords or keyword phrases, followed by your brand name. Unless yours is a highly-recognizable company or service that people will search for by name, do not lead with it in your title meta tag.

    - Write your title meta tag in 6 to 12 words, limited to no more than 63 characters. Yes, spaces count.

Don't look for a complete sentence in your title meta tag, just a string of words that gives search engines and visitors an idea of what you do and who you are while also, of course, incorporating your keywords or keyword phrases. For instance, “News and Commentary Blogs at Writing In The Light” or “News and Commentary Blogs – Writing In The Light” using a hyphen to separate your keywords from your brand name.

SEO-wise the only effect of the description is to draw in visitors from the SERPS.

More often than not the snippet shown in SERPS is the description text. Therefore this text should be appealing to people searching.
