    I have just moved my Blog to my own domain , Is it Possible to Put meta tag on individual page

    Yes dear of course you can put meta tags in each page wise
    Do you have FTP OR Admin panel?

    Ofcourse, edit the page and add meta tag. If you're using some CMS like Wordpress then install plug-in to do the same.
