    If domain.com
    and domain.com/
point to same page, is it necessary to make a 301 redirect to avoid duplicates or protect value?

    domain.com
    and domain.com/

    They could be different pages (a n d a l i n k s e l l e r i s a b u s i n g t h i s).

How can you check which file is being used in
    domain.com
    and
    domain.com/
    index.html, or default.php ... ?

I believe all requests to www.domain.com are automatically routed to www.domain.com/ by the server without the use of a redirect. As far as I'm aware, there is no effective difference.

I assume you are asking which file is being used by the SE's? A great way to check that is by doing a search on the SE's that you are concerned with ;)
And see what they use in the SERP to determine...
