making a search form to search multiple sites

Does anyone know how to make one of these? Would I need to use javascripts? php? I'm running a php enabled linux server with mysql database from godaddy. I already have a forum installed and I want to make one page of my site a meta search. I want people to be able to search maybe 10 or 14 sites from my form.
