Making A Contact Text Field

Hello,

I want to implement a text field where people can send emails, I want to add it to a HTML page.

So basicaly I want to have it so people enter in:

Name:
Email:
Subject:

And then obviously a box below for the message.

Could anyone explain how I would do such a thing?
