Mailing script error

I created two mailing scripts for a site one works and one doesnt. They are both the same except one is longer than the other. All I get when I submit the form is a page not found error. I am by far not a pro at php I am just beginning, so can someone help me out please.

 

Can you send the exact error and both scripts are on same server and directory?

Without knowing script and error its not possible to help.

I get the page cannot be found screen

I am suppose t get a page that shows the answers to the questions on the form and it doesnt send the email either.
