While low CTR does no harm, very low like 0.04% does harm you. The reason is that the site maybe using autosurf, machine-generated, or cheap 1-million-visitors-for-10-buck impressions. And it's strictly against the TOS which prohibits using such traffic program. Read the TOS! And if you want to know the motive that Google bans such traffic, I can tell you that there is not only click-fraud but traffic-fraud too. That's right generating invalid impressions will get you in big trouble too. If you're sure that your visitors are real human that has real intention to view your site, it might be ok. But remember there're crazy people attacking others without any reason at all. These people, who want you banned, use software to generate lots of impressions or clicks for you. You're perhaps being attacked. Dropping adsense support a line wouldn't hurt. Tell them you're suspicious of the traffic. Edited: I didn't know OP was already banned when I posted this.

The reason for you to get banned cant be due to low CTR. If your site is that huge, you can easily find a replacement.
How to get reinstated from Adsense? From anecdotes found online, it seems that the only way is to send in a well-researched email with supporting information to Google and pray. I know a lot of people who can register on Adsense even if they are banned basicaly by : -Registering with your sister's name for example . -Registering after a long time (google deletes the logs every 5 years for example) -Travel to another country (sure if they don't already have your ssn number thats good for me am even not in the us) but the most important thing is to get a new domain , a new mail address , a new brain. good luck
I am not worried about getting back with Adsense or not. However, I am worried and pissed about the fact they did not pay me what I earned without any proof.
