Buying Looking For Someone To Code A Form For Me

    I need a custom form made that will send me the form results to my email. I am wanting to use this in wordpress. I plan on having the following options:

    1) Name
    2) Email Address
    3) Website Name
    4) Website URL
    (These 4 are all input my the client)
    5) Package (Dropdown box with various packages)
    6) Special Instructions (A Large Text Field where client can input any special instructions)

    When submitted, I would like this sent to my email. This is going to be on a wordpress themed website.

    I would also like you to include a mini-tutorial just in case I need to add more options of one these option types. If anyone is interested in doing this, please PM me with your bid and what experience you have in making forms. Thank you so much!
